How it works
- Bill per hour = rate per unit x 60 / unit minutes (or the hourly bill rate).
- Wage per hour = pay rate x (1 + overtime share x (multiplier - 1)). With 5% of hours at time and a half, each hour costs 2.5% more in wages.
- Cost per hour = wage x (1 + burden %) + travel and mileage per hour.
- Gross margin = bill per hour - cost per hour, and as a share of the bill rate.
- Bill rate for a target margin = cost per hour / (1 - target %).
Gross margin still has to pay for the office, scheduling, supervision, training, insurance, software and unbilled time such as cancellations and missed visits. Payroll burden varies a lot by state and by benefits; ask your accountant for your own figure. Public payer rates are set by the payer, so the target-rate line is most useful for private-pay and commercial pricing.
